POSITION PURPOSE:
Provide social work services to patients and families in the acute care setting to meet their medically related social and emotional needs as they impinge on their medical condition, treatment, recovery, and safe transition from one care environment to another using an interdisciplinary approach and coordination of timely discharge planning.
INTERPERSONAL RELATIONSHIPS:
Communicates effectively with patients, families, visitors, co-workers, physicians, and other employees.
Works cooperatively with community agencies PHYSICAL REQUIREMENTS:
Physical health sufficient to meet the ergonomic standards and demands of the position and its location.
SUPERVISORY
Responsibilities:
May supervise students and/ Bachelor's level social work staff.
REPORTING RELATIONSHIPS AND LEVEL OF AUTONOMY:
Supervision as required by licensing regulations by the Social Work Manager.
Clinically responsible to the Social Work Manager, administratively responsible to the Director of the Case Management Onboarding Process:
Please be advised that candidates must successfully complete a background check and pre-employment health screening which includes a drug screen.
EDUCATIONAL REQUIREMENTS:
-Master's degree in Social Work.
-Able to read, write and speak English EXPERIENCE REQUIRED:
-One year experience in an acute inpatient hospital setting or equivalent experience.
PROFESSIONAL LICENSURE AND CERTIFICATION REQUIRED:
